Provides health care supervision and socialization for any adult, frail or disabled, primarily ages 50-90+. Primary purpose is the promotion of optimum health prevention of disease and maintenance of participant's ability to continue residence at home. Provides rehabilitation therapies including physical, occupational and speech services include dental, ophthalmology, podiatry, psychiatric, medical,nursing and nursing rehabilitation social services include medicaid recertifications, medicaid applications, housing assistance and other entitlements, recreational therapy, and non-denominational spiritual care. Door to door transportation, provided for services. Call or walk-in for more information.
Transportation provided for clients. Residents of towns of Babylon, Brookhaven, Huntington, Islip, and Smithtown.
Serves the frail elderly and chronically ill or disabled adults who require long term care services, but are able to remain in the community.
