Single Point of Access (SPOA) for all Care Management, Assertive Community Treatment (ACT) Team and Assisted Outpatient Treatment (AOT) referrals for adults with severe mental illness residing in Suffolk County. Adult SPOA coordinates closely with Pilgrim Psychiatric Center, Mobile Crisis Team, Stony Brook CPEP, community hospitals, and local community mental health agencies to insure recipients obtain access to Care Coordination services in a timely fashion. Adult SPOA oversees the Suffolk County Intensive Case Management (ICM) Program which serves people with severe mental illness and Transitional Care Management (TCM) Program currently operated By EAC. TCM's assist mentally ill inmates, for a smooth transition out of the Suffolk County Jail or NYS prisons, upon release back into the community. Adult SPOA also contains a SPA Housing Coordinator who oversees referrals for Suffolk County residents living with mental illness who are in need of housing, and works closely with community housing agencies.
Diagnosis of mental illness provided by psychiatrist/medial doctor.
